Pay: $18.00 – $21.00 per hour based on experience

Are you a compassionate, highly qualified, and skilled Certified Nursing Assistant (CNA) with a love for providing outstanding care and customer service? Do you want to work on the beautiful Northside of Chicago in a 5 Star residential community while making a difference in peoples’ lives? If so, then The Selfhelp Home, a Continuing Care Retirement Community wants you! We are seeking full-time or part-time  positions in our skilled nursing area. 

The Selfhelp Home is a not-for-profit Jewish retirement community on the north side of Chicago with a tradition of caring that is celebrating over 70 years. JOB DESCRIPTION:

Candidate must have a positive attitude with a good bedside manner, sound judgement, basic nursing skills and knowledge of infection control; be a team player; be able to multitask and work under pressure; possess understanding of caring for residents with dementia.

REQUIREMENTS:

Job requirements are as follows: 

All candidates will be required to pass a criminal background check, drug screen and physical exam.

    • Have satisfactorily completed an Illinois-approved CNA training program and passed the written competency test 
    • Meet all State requirements as stipulated in the Health Care Background Check Act Registry to work as an Illinois CNA in a long-term care facility
    • Must have current CPR certification 
    • Excellent written and verbal communication skills
    • Must have basic computer knowledge and be able to operate equipment, as needed, in a safe manner
  • High level of professionalism and customer service

JOB DESCRIPTION:

The primary role of the staff nurse is to provide direct nursing and related services to attain or maintain the highest possible physical, mental, and psycho-social well-being of each resident, as determined by resident assessments and individual plan of care in his/her assigned unit. He/she assures quality of care by adhering to the therapeutic standards; measuring clinical outcomes against resident’s care goals and standards; making and recommending necessary adjustments; following facility and department philosophies and standards of care set by the professional nursing standards and, the governing agency regulations. He/she manages and directs care staff under guidance of the residents assigned to the unit

REQUIREMENTS:

Job requirements are as follows:

  • Current State of Illinois RN license
  • At least 1 year of experience in long-term care, rehabilitation, assisted living or memory care facility 
  • Knowledge of nursing professional standards, state and federal regulations pertaining to nursing services.

  • High level of professionalism and customer service

Job Description:

  • Plans, develops and implements the policies of the Assisted Living department, which include, admission of new residents, initiate and update service plans, manage medications for residents receiving medication assistance.
  • Reviews Medication Delivery for Accuracy
  • Resident charting in current software
  • Compliments and helps implement resident plan of care developed by the Assisted Living director and resident
  • Responds to resident medical emergencies with appropriate action and documentation in resident record
  • Coordinate with personal aides and CNAs to ensure resident care needs are met in accordance with individual care plans
  • Assists with resident appointment scheduling, escort and transportation arrangements when necessary
  • Solicits resident feedback and coordinates with resident, family and assisted living director in problem solving, establishing and updating individual service plans
  • Must be vaccinated with the latest Covid19 booster
  • Administer and pass medications as well as give medication reminders and oversee medication self-administration.
  • Other duties as assigned

Job Description Responsibilities:

  • Works as Front Desk Assistant during downtime from driving
  • Assists residents with any special needs during transportation, such as carrying packages, opening doors, assisting residents onto and out of the vehicle and to the point of destination
  • Provides miscellaneous pickup and delivery services as needed
  • A report in writing of any mechanical problems with the vehicle that is being used by you
  • Review daily the Driver’s Daily Schedule (this document indicates driver’s daily assigned schedule (driving tasks)
  • Report all incidents with the vehicle
  • The driver is to be aware of his entire environment (inside the bus and outside of the bus).

If an unusual occurrence that seems to be an emergency or if it is an emergency, the following procedure is to be adhered to:

  • Secure the vehicle by parking to the side (or in the most appropriate safe place).
  • Call 911 and inform the emergency of the situation and where you are located
  • Follow emergency procedures as necessary.
  • Call Selfhelp Home to notify the Executive Director

Completes and maintains the following:

  • Vehicle Inspection Sheets (the sheet that verifies that vehicle is in proper operating conditions)
  • Inspects vehicle that will be used for assignment assuring that it is maintained in good working condition.
  • Maintains cleanliness of vehicle
  • Work the occasional weekend and or holiday
  • Ensures all visitors and staff sign into the system
  • Performs rapid COVID tests on all visitors and vendors as required
  • Signs out and delivers packages to residents and departments as needed
  • Covers front desk as needed
  • Other jobs and tasks as assigned

Qualifications:

  • Must be at least 21 years of age
  • Must have appropriate driver’s license with an acceptable driving record in accordance with Federal Department of Transportation (DOT) and state regulations
  • Must be knowledgeable of all safety precautions and comply with safety procedures
  • Ability to react calmly in emergency situations and be considerate
  • Ability to operate a chair lift and secure regular and mechanical wheelchairs and scooters
  • Ability to work semi-independently without direct supervision by following all community procedures and the ability to follow through on assigned tasks and demonstrate initiative.
  • Will train to perform COVID tests and all required front desk tasks
  • Computer skills required
