Each resident is an individual with unique needs and preferences. We meet with the resident and family to evaluate the need for assistance and support in various areas. This evaluation helps determine the appropriate care and service that preserves each resident's dignity, encourages independence and best enables freedom of choice. You can remain in your apartment and receive the services you need. Assisted living services are in addition to the monthly apartment rates.
Three (3) levels of assisted living care are offered at The Selfhelp Home:
LIGHT: LEVEL ONE
Medication Management or
Bathing/ Stand-by assist
Cueing
Minimum level of assistance with any ADL (Activities of Daily Living)
TED Hose
MEDIUM: LEVEL TWO
Medication Management and
Bathing 3 or less times a week
Assistance with Transfers
Daily Bed Making
Weekly Personal Laundry
HEAVY: LEVEL THREE
Bathing 4 or more times a week
Medication Management
Assistance with most ADL's, minimally
Continence Care
Escort regularly
Transfer assistance
Daily Bed Making
Weekly Personal Laundry
INCLUDED WITH ASSISTED LIVING AT ALL LEVELS ARE:
Three meals per day
Emergency call system
Escort Services inside facility to meals and activities
All utilities, except telephone
Security provided 24 hours each day
Weekly housekeeping and linen service
